Class 1 – Who we are and what we do

Practice details, information about our Practice team and our opening times are all published on our website. This information can be accessed via the About Us page. 

The practice aims to follow National Institute for Health and Clinical Excellence (NICE) and National Service Framework guidelines.

Class 2 – What we spend and how we spend it

The Practice receives money from NHS Wales according to its contract for national General Medical Services in exchange for services provided for patients. This information can be obtained on request in writing from the Practice Manager. 

There may be circumstances where material cannot be released because it is confidential or commercial information or the appropriate officer designated for these purposes, under the Act, has taken the view that it may be prejudicial to the conduct of the Practice’s affairs.

If this is the case, we will respond to your FOI request with a formal letter acknowledging the reasons why we are unable to give you this information.
 

Class 3 – What our priorities are and how we are doing

We work with the Merthyr Tydfil Primary Care Cluster which comprises of 6 practices across the North Merthyr.  The practice meets regularly to discuss the provision of services in the locality and we contribute to the annual cluster delivery plan, services to be implemented and demand and capacity data.

All practices have reviews by Health Inspectorate Wales and the Health Board.  We have no recent reports to publish.

Class 4 – How we make decisions

The Partners and Practice Manager meet regularly to review practice operational and strategic plans.  The Practice Manager also meets the nursing team on a regular basis to discuss operational matters and future plans.

Decisions are also made in the cluster meetings.

Class 5 – Our policies and procedures

Our policies and procedures are published on our website. These can be accessed via the Practice Policies page.
