Referrals and Specialist Care

When you need specialist assessment, your clinician will discuss this with you and arrange an appropriate referral.

We work closely with NHS Wales specialist services and other healthcare professionals to ensure you receive the right care.

Annual Health Reviews

We provide a range of annual reviews to help monitor health, identify any changes early, and ensure patients receive the right support and care.

Some patients will be invited for these reviews as part of routine practice care. If you or someone you care for would benefit from a review, please contact the practice for further information.

Dementia Reviews

We offer regular reviews for patients living with dementia to support their ongoing health and wellbeing.

These reviews provide an opportunity to:

We recognise that dementia affects both individuals and those supporting them, and we aim to provide a person-centred approach that considers the needs of both patients and carers.
