If you need a fit note (sometimes called a sick note or Med3), please submit a request using our online consultation system or by contacting the receptionist.
All requests are reviewed by a clinician. Depending on the information provided and your medical history, we may:
A fit note can only be issued where there is a valid medical reason and the condition is supported by information in your medical record.
You do not usually need a fit note for the first 7 calendar days of sickness absence. During this period, you can self-certify your illness with your employer. The fit note: guidance for patients and employees - GOV.UK
If you have been off work for more than 7 calendar days, you may require a fit note.
If your absence from work relates to:
the fit note should normally be provided by the hospital clinician responsible for your care. Please contact the hospital department directly in the first instance.
The length of a fit note is based on clinical judgement and the nature of your condition.
For most new illnesses, fit notes are not usually issued for more than 3 months at a time. Longer periods may be considered for long-term conditions where appropriate.
A fit note may be backdated if the clinician is satisfied that there is sufficient medical evidence to support this.
Fit notes cannot be issued for future periods of sickness that have not yet been clinically assessed.
Once completed, your fit note will normally be sent electronically to you by secure email or through our online services, where available.
