We offer a range of additional services to support your health, wellbeing, and social needs. Some services may contact patients directly based on health reviews or searches of medical records, while others are available for you to access if you feel you would benefit.

Physiotherapy

If you have muscle, joint or back pain, you may be able to refer yourself directly to physiotherapy.
Physiotherapists can help with problems such as:

  • Back or neck pain

  • Joint pain

  • Sports injuries

  • Mobility problems

Self-referral can often be quicker than waiting for a GP appointment.


Mental Health Support

If you are feeling overwhelmed, low in mood, anxious, or finding it difficult to cope, you are not alone. Support is available, and we are here to help you find the right care.

Many people experience emotional or mental health difficulties at times, and reaching out is an important first step. You can contact the practice to talk about how you are feeling, and we will guide you to the most appropriate support.

Depending on your needs, we may offer:

  • An appointment with a GP
  • An appointment with a Primary Care Mental Health Nurse
  • Advice on self-help and online resources

Health & Wellbeing Support

You may be offered the following routine health checks or as part of your ongoing care—for example, if you are identified as being at increased risk of diabetes or cardiovascular disease. You do not need to wait for a health check or referral to ask for support. If you would like help with your health or wellbeing, please speak to the practice team.

Our health and wellbeing support can include:

  • Prediabetes and diabetes prevention
  • Healthy weight and weight management
  • Stopping smoking
  • Healthy eating and nutrition
  • Physical activity and exercise
  • Improving general health and wellbeing
  • Reducing your risk of long-term health conditions

WISE – Wellness Improvement Service

WISE, Wellness Improvement Service| Merthyr Cluster 

WISE services focus on improving overall wellbeing and supporting people to manage their health more effectively. They can help with:

  • Lifestyle changes
  • Emotional wellbeing
  • Long-term condition support
  • Connecting you with local community resources

Cardiovascular Disease (CVD) Clinics

Our CVD clinics aim to reduce your risk of heart disease and stroke. These clinics may be offered proactively if you are identified as at risk. Support includes:

  • Blood pressure and cholesterol management
  • Lifestyle advice
  • Medication reviews
  • Risk assessment and prevention planning

Lifestyle Advice & Weight Management Services

We offer support to help you make sustainable lifestyle changes. This includes:

  • Weight management programmes including referral to secondary care for specialist assessments.
  • Healthy eating and diet advice
  • Physical activity and exercise guidance
  • Smoking cessation support

 


Support for Carers

We recognise the vital role that carers play in supporting others. If you look after a family member, partner, or friend due to illness, disability, or frailty, you may be entitled to additional support.

Information for carers and the people they support - Public Health Wales

Our practice has a Carers' Champion who can:

  • Help identify if you are a carer
  • Provide information on available support and services
  • Guide you on accessing benefits and financial support
  • Offer advice on local resources and wellbeing support

As a registered carer, you may also be eligible for:

  • Relevant NHS Wales health checks
  • Recommended immunisations (such as seasonal flu vaccines)
  • Signposting to carers’ support organisations

Support from CAB: Citizens Advice Merthyr Tydfil - Cyngor ar Bopeth Merthyr Tydfil

The Citizens Advice service provides free, confidential, and impartial advice. They have a dedicated CAB team member working with the practice every Friday all day from 09:00 to 16:00.  They can support you with:

  • Financial concerns and debt advice
  • Housing issues
  • Employment and benefits guidance
  • Social and practical aspects of care

This service can be especially helpful if your health is affected by social or financial difficulties.
